In order to receive the early bird rate, registration and payment must be received by Jack Hirose & Associates' office on or before the cut-off date. Received cheques must be postmarked on or before the cut-off date.
Payment can be made via cheque, VISA, or Mastercard. Sorry, we do not accept purchase orders or American Express.
Canadian Psychological Association, Canadian Counselling and Psychotherapy Association, Canadian Addiction Counsellors Certification Federation, Association of Social Work Boards, Canadian Association of Rehabilitation Professionals, Canadian Professional Counselling Association, Employee Assistance Certification Commission, College of Family Physicians of Canada (MainPro M1.) Approved for up to 30 credit hours (four day attendance,) 7 credit hours per day, 9 credit hours on day one (pre-conference.)
To be eligible for group rates registrants must register through their assigned "Group Contact" person. All group members must be submitted together at time of registration by the group contact. The "Group Contact"
person must contact our office directly by phone 1-800-456-5424 or by email at registration@jackhirose.com. Separate payment may be submitted by each participant. Please specify if one group receipt is sufficient or if separate receipts are needed. Group rates only apply to registrations for all 4 days.
Letter from institution verifying full time student status required. Student ID cards are not acceptable proof of full time student status. Student rates only apply to registrations for all 4 days.
If you are unable to attend, you are invited to send an alternate in your place, at no extra cost. Please notify us of the name of the alternate, so that we may correct our records and minimize confusion at check in.
Refunds will be available, minus $40 administration fee, 14 days or more prior to the Conference. Alternatively, you could receive a credit, minus $40 administration fee, towards future Jack Hirose & Associates Inc. training. For cancellations less than 14 days prior to the event, a credit will be available (less $40 admin. fee) towards future Jack Hirose & Associates Inc. training. Non-attendance at the Conference will not be grounds for a refund.
In the unlikely event that we have to cancel the Conference, we will issue a full refund for fees.
Conference seats cannot be shared between individuals.
Our liability is limited to refunds for Conference fees only. Jack Hirose & Associates Inc./EDGEWOOD Treatment Centre reserves the right to cancel the event. Please make hotel & travel arrangements with this in mind.
Jack Hirose & Associates/EDGEWOOD Treatment Centre is not responsible for any presenter’s or participant’s statements, acts, materials, or omissions. Jack Hirose & Associates/EDGEWOOD reserve the right to replace guest presenters due to unforeseen circumstances. The use of audio and video taping devices, beepers, and cell phones is not permitted at the Conference. Children and unregistered guests are not permitted in the meeting rooms.
